Wraparound Case Management

We coordinate legal, medical, physical, social, personal care, and psychological services for youth and families to streamline methods and ensure achievement of developmental priorities.

Youthcentrix clients who receive individual and direct services are automatically eligible for enrollment in our monitored case management process.  Contact our office using the enrollment information form below to share your needs with a case manager.
